Student Nomination Form
Thank you very much for taking the time to nominate a student employee for the Student Employee of the Award. By taking this initiative, you are acknowledging the service your student provides to not only your office, but to the University as well. Below you will see a list of requirements before you fill out the actual form. Please read and make sure your student fulfills each requirement that is specified.

Requirements:
- Student must be a continuing student in the Spring ’09 semester.
- Student must be under the F1 or F3 pay code, which denotes the student as a student employee of the University of Hawai‘i under the Center for Career Development & Student Employment (CDSE).
- Student must also be enrolled as a UHM classified student with the following minimum amount of credits: 9 credits for Undergraduates; 6 credits for Graduates.
- Must work a minimum of 10 hours per week during the academic periods.
- Must be in good academic standing.
- Must also be employed at least 6 months prior to the time of nomination.
Nominator's letter of recommendation required for all Student nominations
After filling out the necessary information about you [the nominator] and the student employee, a letter of recommendation is required by the nominator. The letter is essential to determine that your student employee stands above all as the Student Employee of the Year and names the representative of the University to the State, and possibly Regional and National, level of the competition. The letter of recommendation has no limit on words for we would like you to freely express your reasons for the nomination.
To help you better present your letter to our Selection Committee, you could encompass your thoughts and feelings using the following six themes:
- Quality of Work
- Reliability
- Initiative
- Professionalism
- Uniqueness of Contribution
- Campus/Community Service
Student's essay required to complete all Student nominations
At the University-level, we require the student nominee to present an essay (500 word max.) that describes their experience as a student employee and answers the following question:
- “What has helped me develop as a person (i.e. personally, professionally, and/or academically) by working as a student employee?”
