Team Nomination Form

Thank you very much for taking the time to nominate a team for the Student Employee-Team of the Year Recognition. By taking this initiative, you are acknowledging the service your students provide to not only your office, but to the University as well.

Important Nomination Information:

  • NOMINATION DEADLINE: February 9, 2009 at 4:00 p.m.

After filling out the necessary information about you [the nominator] and each student employee, a letter of recommendation is required by the nominator.

Nominator's letter of recommendation required for all Team nominations

The letter is essential to determine that your team stands above all as the Student Employee-Team of the Year. There is no limit on words for we would like you to fully express your reasons on this nomination. In your letter please briefly explain the duties of the team as a cohesive unit and include your reasons for their nomination.
